#98 - Geriatric Team: Selayang Hospital Experience Adlin Natasya bt Mohd Said

There is a lot of evidence regarding the benefits of having multidisciplinary team meetings in managing patients. It positively helps in achieving an optimal outcome for the patients.
In Selayang Hospital, Geriatric team was first initiated by Consultant Geriatrician in March 2013. It started with only 2 beds in Medical Ward 10C and initial rounds were done by consultant geriatrician, specialist and therapist. In June 2014, 6 beds in Palliative Ward 4C were allocated for Geriatrics. The team then expanded and Medical Officers, Physiotherapist, Occupational Therapist, Dietitian, Social Worker and also nurses were included. Later, more patients were referred to the team mainly for rehabilitation since there is no any other rehabilitation facility/services in the hospital. Total number of patients seen in 2016 was about 107 patients with age ranging from 21 years old to 86 years old.
The team would have Multidisciplinary Team Meeting (MDT) weekly. Routine screening done using MNA (Mini Nutrition Assessment) by the dietitian, the Modified Barthel Index by the Occupational Therapist and also Mini Mental State Examination (MMSE) by doctors. We also successfully co-organized Selangor Geriatric Conference in 2016 with Tengku Ampuan Rahimah Hospital and Sg Buloh Hospital. Our team also do home visits, and any other rehabilitation related activities such as ‘Sukaneka’ and cake decorating for patients. Activities are designed to create better environment for recovery and better mental health among patients.
With routine multidisciplinary meeting, the team could establish better understanding of the patients’ medical condition, mental health and physiological need. Thus, facilitate an effective platform to achieve consensus and to develop patient-centered goals resulting in better outcome for the patients.
